ABSTRACT

ABSTRACT Objective To evaluate the standards of practice of Brazilian cataract surgeons in relation to the protective measures adopted to mitigate the risks of transmission of COVID-19 during cataract surgery, in asymptomatic patients. Methods A descriptive, cross-sectional, quantitative paradigm study, developed from a self-administered electronic questionnaire sent to ophthalmologists and residents/specialists in ophthalmology in Brazil, who performed cataract surgeries in 2019 and 2020, connected through social media and mail listing from local societies. Results Of the 303 participating surgeons, 159 (n=52.2%) performed elective cataract surgeries between March 20th, 2020 to June 1st, 2020. Among the measures adopted by ophthalmologists with the purpose of preventing viral transmission, the patient's temperature was measured by 84.3% (n=134), and the verification of respiratory symptoms and contact/exposure to cases of COVID-19 by 87.4% (n=139). Most did not submit their patients to laboratory tests to detect COVID-19 (145; 91.2%). In surgery, 44.7% (n=71) used an N95 mask, and 69.2% (n=110) kept their patients with a mask. No stage of phacoemulsification was modified in 144 (90.6%) participants, 13 (8.2%) added methylcellulose under the main incision, and two (1.3%), modified another surgical stage. Conclusion The COVID-19 pandemic significantly interrupted part of cataract surgeries in Brazil from March to June 2020 and measures to prevent viral spread are being heterogeneously adopted by surgeons. Understanding these measures could be the first step to improve strategies to return to pre-pandemic levels.

RESUMO Objetivo: Descrever o perfil clínico, cirúrgico e geográfico de pacientes acompanhados em um hospital universitário e submetidos a transplante de tecido corneano penetrante, com descrição das indicações para o procedimento e do tempo médio na fila de espera. Métodos: Estudo transversal e retrospectivo, incluindo 40 olhos de 40 pacientes submetidos à ceratoplastia penetrante no período de 1° de janeiro de 2018 a 31 de dezembro de 2019, acompanhados em um hospital universitário em Alagoas. Os dados foram coletados por meio das fichas de informações cirúrgicas dos transplantes de córnea da Central de Notificação, Captação e Distribuição de Órgãos e Tecidos de Alagoas, adaptados aos objetivos da pesquisa. Resultados: Dos transplantes de córnea estudados, 52,5% (n=21) foram realizados em pacientes do sexo feminino, 62,5% (n=25) na faixa etária acima de 60 anos, com média de idade de 59,17 anos (±20,4). Todos ocorreram em pacientes que residiam no estado de Alagoas, sendo 60% deles na região intermediária de Maceió. As principais indicações para o transplante de córnea foram ceratopatia bolhosa do pseudofácico (n=11; 27,5%), descemetocele (n=9; 22,5%) e falência tardia do enxerto (n=6; 15%). Dentre os procedimentos, 70% foram realizados no olho esquerdo (n=28) e 65%(n=26) com propósito óptico; houve associação de extração de catarata em 22,5% (n=9), e 5% (n=2) apresentaram complicações peroperatórias. O tempo médio geral em lista de espera foi de 332,3 dias (11 meses), sendo 486 dias (2 anos e 4 meses) para cirurgias eletivas e 12,8 dias para as de urgência. Conclusão: O tempo de espera para cirurgias eletivas foi longo e inadequado. A principal condição indicadora para o transplante de córnea foi a ceratopatia bolhosa. O conhecimento do perfil dos transplantes de córnea pode permitir a identificação de grupos de risco para fins de prevenção e implementação de cuidados, que resultem em prognósticos mais favoráveis, bem como incentivar a implementação de políticas internas e externas para melhoria do sistema captação-doação.


ABSTRACT Objective: To describe the clinical, surgical, and geographic profile of patients submitted to corneal transplantation and followed up at a teaching hospital, and, with a description of the indications for the procedure and mean waiting time. Methods: A cross-sectional and retrospective study, including 40 eyes of 40 patients who underwent penetrating keratoplasty, from January 1, 2018 to December 31, 2019, followed up at a teaching hospital in Alagoas State. Data were collected using the surgical records of corneal transplants, from the Central Reporting, Procurement and Distribution of Organs and Tissues of Alagoas, and adjusted to the research objectives. Results: Of the corneal transplants studied, 52.5% (n=21) were performed in female patients; 62.5% (n=25) in the age group over 60 years; with a mean age of 59.17 years (± 20.4). All transplanted patients lived in the state of Alagoas, 60% of them in the intermediate metropolitan region of Maceió. The main indications for corneal transplantation were pseudophakic bullous keratopathy (27.5%, n=11), descemetocele (22.5%, n=9) and late graft failure (15%, n=6). Seventy percent (n= 28) of procedures were performed on the left eye and 65% (n=26) for optical purposes. There was an association of cataract extraction in 22.5% (n=9), and 5% (n=2) had perioperative complications. The mean waiting list time was 332.3 days (11 months); in that, 486 days (2 years and 4 months) for elective surgeries and 12.8 days for emergency surgeries. Conclusion: The waiting time for elective surgeries was long and inappropriate. The major indication for corneal transplantation was bullous keratopathy. Knowledge of the clinical profile of corneal transplants can enable identifying the risk groups for prevention and implementation of care, resulting in better prognosis, fostering implementation of internal and external policies to improve the procurement-donation system.

Resumo Objetivo: Caracterizar o perfil dos egressos de Programas de Residência Médica em Oftalmologia no Estado de Alagoas, quanto à sua formação e prática profissional. Métodos: Estudo descritivo, retrospectivo, de paradigma quantitativo, desenvolvido a partir de questionário autoaplicável enviado aos médicos egressos do programa de residência em Oftalmologia do Hospital Universitário Professor Alberto Antunes (HUPAA/UFAL) que finalizaram a residência nos últimos 10 anos. Resultados: Dos 28 ex-residentes, 27 responderam ao questionário. Foram predominantemente mulheres (70,4%), jovens (média 35,92±4,05 anos), de naturalidade alagoana (51,8%), com graduação em instituições públicas (81,5%). A maioria se fixou em Maceió e região metropolitana (51,8%) e trabalha em ambulatórios privados (92,6%), com pouca atuação na gestão (14.8%) e docência (7,4%). Mais da metade não possui titulação de especialista pelo Conselho Brasileiro de Oftalmologia (59,3%) e realizou programa de complementação especializada tipo "fellowship" (74%). Apenas 7,4% realiza atividades em pesquisa e 3.7% possui mestrado. No geral, os egressos estudados estão satisfeitos com o exercício da especialidade (100%) e com sua formação na instituição (86,3%), sugerindo melhora na assistência cirúrgica e ensino teórico (29,7%). Conclusão: A avaliação da realidade e atuação profissional dos egressos e de suas percepções foi capaz de levantar o perfil de escolhas profissionais e condições de inserção e satisfação profissional. Também permitiu avaliação externa complementar da instituição formadora, fornecendo subsídios para melhoria da qualidade dos programas de residência médica em Oftalmologia.


Abstract Objective: To characterize the profile of graduates of Medical Residency Programs in Ophthalmology in the state of Alagoas, regarding their training and professional practice. Methods: Retrospective descriptive study developed from a self-administered questionnaire sent to doctors who graduated from the Ophthalmology residency program at the Professor Alberto Antunes University Hospital (HUPAA / UFAL) who completed their residency in the last 10 years. Results: Of the 28 former residents, 27 responded to the questionnaire. They are predominantly women (70.4%), young (average 35.92 ± 4.05 years), born in the State of Alagoas (51.8%), graduated from public institutions (81.5%). Most settled in Maceió and the metropolitan region (51.8%) and work in private outpatient clinics (92.6%), with little experience in management (14.8%) and teaching (7.4%). More than half do not have a specialist degree from the Brazilian Council of Ophthalmology (59.3%) and carried out a specialized fellowship complementation program (74%). Only 7.4% carry out research activities and 3.7% have a master's degree. In general, the studied graduates are satisfied with the exercise of the specialty (100%) and with their training at the institution (86.3%), suggesting an improvement in surgical care and theoretical teaching (29.7%). Conclusion: The evaluation of the reality and professional performance of the graduates and their perceptions was able to raise the profile of professional choices and conditions of insertion and professional satisfaction. It also allowed complementary external evaluation of the training institution, providing subsidies for improving the quality of ophthalmology medical residency programs.
